APP-04 - Petroleum - Consent to change operator or transfer interests
Use this form to apply for consent to change operator or transfer permit participant interests.
Please reference the Petroleum Application Guide to support the completion of your application form. It identifies the additional information to be provided to make a complete application. Should your application be incomplete, it may be rejected.

You need to apply for Ministerial consent to the transfer of participating interest in the permit if the percentage holdings of the permit are changing (such as where a permit participant is being added or removed from the permit). (Every permit must have a permit operator. The operator is the permit participant who is responsible for the day to day management of activities under the permit). An executed copy of the agreement that requires consent must accompany the application. Applications must be made within three months of the date of the agreement, otherwise a new agreement will need to be signed. If you are applying for Ministerial consent to a change of operator and the new operator is not currently a permit participant you will also need to apply for consent to a transfer of interest. |

Note: if there is more than one proposed permit participant, APP-04 must be accompanied by:
- a copy or copies of APP-09 Additional permit participant information application form relating to each additional proposed permit participant.
- a copy or copies of APP-10 Application authority form for each proposed permit participant that is not you as an individual.
If you want to submit the application online, you will also need authority to view the permit information online. You will already have this if you are the Permit administrator or a participant in the permit.
