This form is to be used to apply for a full surrender of a permit only. If you are only surrendering part of a permit, please use form APP-02. Please note that surrenders take effect on the date the application is accepted, that is when the Minister makes the decision, not on the day the application is lodged with NZP&M. The surrender of a permit does not release the permit participant from any liability in respect of the permit up to the date of the surrender, or any act under the permit up to the date of the surrender giving rise to a cause of action. Surrenders may not be accepted where there is money or reports owing on the permit. |

How to apply
You need to provide a completed application form and supporting information.
Fill out your application form online or email it to us
Apply online using the online permitting system. To do this, you will need authority to view the permit information online. You will already have this if you are a current permit participant, or a permit administrator.
Or complete a full surrender application form (App-03) and email it to us at nzpam@mbie.govt.nz.

Note: The form must be accompanied by a copy or copies of APP-10 Application form for each permit participant that is not you as an individual.

Compile any supporting documents
You need to provide supporting information with your permit application. Your application may be delayed or rejected if you don’t provide all the information needed to support your application.

We suggest you complete the template and attach it to your online application or email it to us alongside your application form.
